Notice of time sensitive tax form completion instructions

What is the objective of this campaign?

This is a service notice to inform a participant that their IRS W-8BEN Form, on record with Fidelity, has expired or is expiring on January 1. The communication includes instructions to complete a new form and return it to Fidelity to continue to be granted tax benefits.

IRS Form W-8BEN is completed by the participant to establish the individual’s status as a nonresident alien and their country of residence. Fidelity uses the information provided on the form to establish if an applicable resident country tax treaty applies to reduce the tax rate. This form is kept on file at Fidelity. The form expires at the end of the third calendar year of the year following the date the form is signed.

  • This annual communication is sent in September of the year in which the individual participant’s form will expire to allow time to complete and return a new form.

Who will receive this campaign?

Participants with a greater than $0 plan balance whose W-8BEN on file has expired or is expiring.
